The foam density should be based on your body weight and the comfort level you are looking for. This mattress being a medium to firm sounds like it is far too firm for you, especially if you are also using a topper. A topper is the best option to soften down the overall firmness in the comfort layer of a mattress if this doesn't change the performance to your liking, then you may need to look at changing the mattress completely.
